Cette ann\u00e9e 2021, la FHU PREMA participe au financement du fonctionnement de l\u2019\u00e9tude <strong>TR<\/strong>ansfert trans-placentaire d\u2019<strong>AN<\/strong>ticorp<strong>s<\/strong> <strong>M<\/strong>aternels fonctionn<strong>e<\/strong>ls an<strong>t<\/strong>i-palus<strong>tre<\/strong>s (<em>TRANsMettre<\/em>), coordonn\u00e9e par C\u00e9lia Dechavanne (MERIT \u2013 IRD).<\/p>\n\n\n\n<p>En cas de paludisme associ\u00e9 \u00e0 la grossesse, les globules rouges infect\u00e9s par le parasite <em>Plasmodium falciparum <\/em>sont s\u00e9questr\u00e9s au niveau du placenta du c\u00f4t\u00e9 maternel. Cette s\u00e9questration est \u00e0 l\u2019origine de cons\u00e9quences cliniques dramatiques pour la m\u00e8re et son nouveau-n\u00e9. M\u00eame si le lien entre paludisme gestationnel et pr\u00e9maturit\u00e9 est tr\u00e8s peu document\u00e9, il est connu que le risque de donner naissance \u00e0 un nouveau-n\u00e9 pr\u00e9matur\u00e9 est augment\u00e9 de 3 fois si 10% des globules rouges pr\u00e9sents dans le placenta sont parasit\u00e9s.<\/p>\n\n\n\n<p>Sur le plan immunologique, il a \u00e9t\u00e9 montr\u00e9 que la pr\u00e9sence d\u2019anticorps anti-palustres fonctionnels chez la m\u00e8re diminuait risque paludisme associ\u00e9 \u00e0 la grossesse, mais le r\u00f4le de ces anticorps dans la protection de l\u2019enfant pr\u00e9matur\u00e9 n\u2019est pas \u00e9tabli. Le projet TRANsMettre vise \u00e0 explorer le r\u00f4le des anticorps maternels fonctionnels passant la barri\u00e8re placentaire dans la protection des enfants lors des premiers mois de vie avec un regard particulier sur les enfants n\u00e9s pr\u00e9matur\u00e9s.<\/p>\n\n\n\n<p>Pour atteindre cet objectif, des nouveau-n\u00e9s r\u00e9sidents au B\u00e9nin \u2013 pays end\u00e9mique pour le paludisme \u2013 ont \u00e9t\u00e9 suivis lors des enqu\u00eates de Tori Bossito (545 nouveau-n\u00e9s dont 10,4% sont class\u00e9s \u00ab&nbsp;moyens pr\u00e9matur\u00e9s&nbsp;\u00bb) et de PlasDCty (70 nouveau-n\u00e9s).<\/p>\n\n\n\n<div class=\"wp-block-media-text alignwide has-media-on-the-right is-stacked-on-mobile\"><figure class=\"wp-block-media-text__media\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"488\" src=\"https:\/\/fhu-premimpact.org\/wp-content\/uploads\/2021\/07\/Schema-fonctionalite-1024x488.jpg\" alt=\"\" class=\"wp-image-4103 size-full\" srcset=\"https:\/\/fhu-premimpact.org\/wp-content\/uploads\/2021\/07\/Schema-fonctionalite-1024x488.jpg 1024w, https:\/\/fhu-premimpact.org\/wp-content\/uploads\/2021\/07\/Schema-fonctionalite-300x143.jpg 300w, https:\/\/fhu-premimpact.org\/wp-content\/uploads\/2021\/07\/Schema-fonctionalite-768x366.jpg 768w, https:\/\/fhu-premimpact.org\/wp-content\/uploads\/2021\/07\/Schema-fonctionalite-1200x571.jpg 1200w, https:\/\/fhu-premimpact.org\/wp-content\/uploads\/2021\/07\/Schema-fonctionalite-1320x629.jpg 1320w, https:\/\/fhu-premimpact.org\/wp-content\/uploads\/2021\/07\/Schema-fonctionalite.jpg 1447w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/figure><div class=\"wp-block-media-text__content\">\n<p>La fonctionnalit\u00e9 des anticorps se mesure, au laboratoire, par leur capacit\u00e9 \u00e0 activer les cellules immunitaires telles que les neutrophiles (Figure 1A) et les monocytes (Figure 1B). Certaines modifications de s\u00e9quences (symbolis\u00e9es par \u00ab\u00a0*\u00a0\u00bb dans la Figure 1) au niveau des anticorps et de leurs r\u00e9cepteurs sont connues pour r\u00e9duire la fonctionnalit\u00e9 et seront \u00e9galement caract\u00e9ris\u00e9es dans ce projet.<\/p>\n<\/div><\/div>\n\n\n\n<p>Ces exp\u00e9riences seront r\u00e9alis\u00e9es au laboratoire de l\u2019Institut de Recherche Clinique du B\u00e9nin (IRCB) et feront l\u2019objet d\u2019un transfert de technologie permettant la formation d\u2019un doctorant b\u00e9ninois sur ces approches. Ces analyses permettront, pour la premi\u00e8re fois, de comprendre le r\u00f4le des anticorps maternels dans la protection contre les infections palustres chez les enfants n\u00e9s
