{"id":9169,"date":"2026-04-16T12:12:07","date_gmt":"2026-04-16T10:12:07","guid":{"rendered":"https:\/\/fhu-premimpact.org\/?page_id=9169"},"modified":"2026-04-16T12:12:10","modified_gmt":"2026-04-16T10:12:10","slug":"vers-un-futur-vaccin-contre-la-meningite-bacterienne-du-nouveau-ne-copie-copie-3-copie-copie-7","status":"publish","type":"page","link":"https:\/\/fhu-premimpact.org\/en\/focus\/vers-un-futur-vaccin-contre-la-meningite-bacterienne-du-nouveau-ne-copie-copie-3-copie-copie-7\/","title":{"rendered":"Une goutte de sang et\u00a0un\u00a0algorithme d&#8217;apprentissage automatique pour mieux pr\u00e9dire les\u00a0complications graves chez les\u00a0pr\u00e9matur\u00e9s\u00a0 &#8211; copie"},"content":{"rendered":"\n<p>WASHINGTON, 23&nbsp;janvier 2026 &#8211; Des chercheurs am\u00e9ricains et canadiens ont d\u00e9velopp\u00e9 et valid\u00e9 une m\u00e9thode de stratification de la sant\u00e9 m\u00e9tabolique des nouveau-n\u00e9s tr\u00e8s grands pr\u00e9matur\u00e9s, en appliquant un mod\u00e8le d&#8217;apprentissage automatique aux m\u00e9tabolites mesur\u00e9s \u00e0 partir des gouttes de sang pr\u00e9lev\u00e9es au talon \u00e0 la naissance, permettant de pr\u00e9dire mieux que les mod\u00e8les existants le risque de survenue de quatre maladies graves fr\u00e9quentes chez ces pr\u00e9matur\u00e9s, selon une \u00e9tude publi\u00e9e mercredi dans Science Translational Medicine.<\/p>\n\n\n\n<p>La pr\u00e9maturit\u00e9 est associ\u00e9e \u00e0 une morbidit\u00e9 et une mortalit\u00e9 importantes, li\u00e9es notamment \u00e0 quatre pathologies graves couramment observ\u00e9es chez les pr\u00e9matur\u00e9s, la dysplasie bronchopulmonaire, l&#8217;h\u00e9morragie intraventriculaire, l&#8217;ent\u00e9rocolite n\u00e9crosante et la r\u00e9tinopathie du pr\u00e9matur\u00e9, rappellent Alan Chang de la Stanford University School of Medicine \u00e0 Stanford (Californie) et ses coll\u00e8gues.<\/p>\n\n\n\n<p>Les classifications standard fond\u00e9es sur l&#8217;\u00e2ge gestationnel, le poids de naissance et l&#8217;exp\u00e9rience clinique du praticien sont insuffisantes pour pr\u00e9dire l&#8217;\u00e9volution de l&#8217;\u00e9tat de sant\u00e9 des nouveau-n\u00e9s pr\u00e9matur\u00e9s. Il est donc n\u00e9cessaire de d\u00e9velopper de nouveaux outils de stratification afin d&#8217;initier la prise en charge la plus adapt\u00e9e le plus t\u00f4t possible, soulignent-ils.<\/p>\n\n\n\n<p>Les profils m\u00e9taboliques des pr\u00e9matur\u00e9s \u00e0 la naissance pourraient livrer des informations suppl\u00e9mentaires sur le risque de complications par rapport aux mesures actuellement utilis\u00e9es.<\/p>\n\n\n\n<p>Les chercheurs ont exploit\u00e9 les pr\u00e9l\u00e8vements de sang s\u00e9ch\u00e9 effectu\u00e9s de mani\u00e8re syst\u00e9matique au talon des nouveau-n\u00e9s \u00e0 la naissance pour le d\u00e9pistage n\u00e9onatal, issus de 13.536 pr\u00e9matur\u00e9s de 22 \u00e0 29&nbsp;semaines, avec l&#8217;\u00e9volution clinique associ\u00e9e, afin de d\u00e9velopper un indice de sant\u00e9 m\u00e9tabolique bas\u00e9 sur les m\u00e9tabolites mesur\u00e9s lors du d\u00e9pistage n\u00e9onatal et sur la clinique, pour stratifier les nouveau-n\u00e9s pr\u00e9matur\u00e9s \u00e0 risque de l&#8217;une des quatre pathologies fr\u00e9quemment rencontr\u00e9es, via un mod\u00e8le d&#8217;apprentissage automatique. Parmi ces pr\u00e9matur\u00e9s, 12.096 ont d\u00e9velopp\u00e9 une ou plusieurs des quatre pathologies et 1.440 ont servi de contr\u00f4les.<\/p>\n\n\n\n<p>Ce nouvel indice de sant\u00e9 m\u00e9tabolique s&#8217;est av\u00e9r\u00e9 plus performant que d&#8217;autres mod\u00e8les d&#8217;apprentissage automatique pour pr\u00e9dire le risque de survenue de ces quatre maladies, constatent les chercheurs.<\/p>\n\n\n\n<p>Une validation externe de cet indice a \u00e9t\u00e9 effectu\u00e9e sur une deuxi\u00e8me cohorte r\u00e9trospective ind\u00e9pendante constitu\u00e9e de 3.299 tr\u00e8s grands pr\u00e9matur\u00e9s n\u00e9s en Ontario (Canada), dont 2.117&nbsp;cas ayant d\u00e9velopp\u00e9 l&#8217;une des quatre maladies consid\u00e9r\u00e9es et 1.182 contr\u00f4les.<\/p>\n\n\n\n<p>&#8220;Cette \u00e9tude a d\u00e9montr\u00e9 que les nouveau-n\u00e9s tr\u00e8s grands pr\u00e9matur\u00e9s peuvent \u00eatre stratifi\u00e9s selon leur niveau de risque sur la base des caract\u00e9ristiques m\u00e9taboliques obtenues dans le cadre du d\u00e9pistage n\u00e9onatal en routine&#8221;, via des mod\u00e8les bas\u00e9s notamment sur les donn\u00e9es utilisant l&#8217;apprentissage automatique, concluent les auteurs.<\/p>\n\n\n\n<p>L&#8217;indice de sant\u00e9 m\u00e9tabolique bas\u00e9 sur le d\u00e9pistage n\u00e9onatal peut am\u00e9liorer les d\u00e9cisions th\u00e9rapeutiques chez les nouveau-n\u00e9s les plus vuln\u00e9rables, \u00e9tant donn\u00e9 qu&#8217;il repose sur des donn\u00e9es d\u00e9j\u00e0 disponibles via le d\u00e9pistage n\u00e9onatal r\u00e9alis\u00e9 en routine dans les h\u00f4pitaux, en Am\u00e9rique du Nord et dans de nombreux pays europ\u00e9ens, d&#8217;Am\u00e9rique latine, d&#8217;Asie et du Pacifique, ajoutent-ils.<\/p>\n\n\n\n<p>(Science Translational Medicine, <a href=\"https:\/\/www.science.org\/doi\/10.1126\/scitranslmed.adv4942\" target=\"_blank\" rel=\"noreferrer noopener\">publication en ligne du 21&nbsp;janvier<\/a>)<\/p>\n","protected":false},"excerpt":{"rendered":"<p>WASHINGTON, 23&nbsp;janvier 2026 &#8211;
