The Prem’IMPACT University Hospital Federation (FHU) brings together clinical teams (gynecology-obstetrics, neonatal pediatrics, psychiatrists and psychologists, medically assisted procreation and hospital laboratories) and research teams (Institut Cochin, Institut Imagine Necker-Enfants-malades, IRD, Institut Pasteur, ESPCI), medically assisted procreation and hospital laboratories) and research teams (Institut Cochin, Institut Imagine Necker-Enfants-malades, IRD, Institut Pasteur, ESPCI), all of whom are developing research projects linked to perinatal inflammation, prematurity and its consequences.
University clinical services
Six university perinatal centers in the Ile-de-France region participate in this network, as well as 2 maternity units in Benin and Senegal. These are the Cochin – Port Royal, Necker-Enfants Malades, Louis Mourier, Armand Trousseau and Robert Debré hospitals of the AP-HP, the GH Paris Saint-Joseph, as well as the CHU-MEL in Cotonou, Benin, and the CHU of Saint Louis, Senegal. These centers represent 30,00 births/year, with over 2,500 premature babies, 220 doctors, 340 midwives and 600 nurses.
Research teams
Internationally recognized, the 24 Prem’IMPACT FHU research teams from a wide range of disciplines (epidemiology, genomics, molecular and cellular biology, pharmacology, microbiology, immunology, physics-chemistry, psychology and social sciences) share innovative concepts, technologies and tools. They collaborate closely with each other and with clinicians to ensure that research directly benefits patients. It is from this pooling that tomorrow’s innovations will emerge. The expertise and size of this consortium will enable FHU Prem’IMPACT to develop ambitious projects on a national and international scale.

Clinical research support structures
These are platforms that implement translational research, i.e. the link between the fundamental research that is essential to all progress, and patient-centered clinical research: the Mother-Child Clinical Investigation Center (CIC) at APHP and the Vaccinology CIC at Cochin-Pasteur, the Clinical Research Coordination Center (PCRC) at Institut Pasteur and the Clinical Research Institute at Benin.

Prem’IMPACT’s objectives
- validate innovative diagnostic and therapeutic tools to better prevent preterm birth and evaluate its consequences,
- decipher the mechanisms that explain parturition and the vulnerability of preterm infants to perinatal inflammation,
- benchmark best clinical practices and accelerate clinical research to improve the quality of care,
- assess mental health and maternal comorbidities in consequences of preterm birth,
- promote families at the forefront of care and perinatal research,
- enable effective integrated teaching with parents & professionals.
Our ability to achieve these goals is ensured by the complementarity of 20 research teams, the quality of the existing platforms, and the large patient recruitment of >33,000 deliveries, including >2500 very premature babies each year.
The added value of the FHU is to strengthen a network of clinical experts in perinatal care and prematurity, create or reinforce interdisciplinary synergies between clinical and research teams from APHP, Inserm, Pasteur Institute and Ile de France Universities, promote academia-industry collaborations, and integrate parents into active care and research.
Prem’IMPACT FHU FIGHTS PREMATURITY
The Prem’IMPACT consortium, the largest in France in the field of prematurity, will promote interdisciplinary research between basic researchers, clinicians, epidemiologists, parents and technology companies, with 6 main objectives:
1. Innovative diagnostic and therapeutic technologies (leads: V. Biran, V. Tsatsaris)
- Identification and validation of biomarkers of prematurity
- Early detection of neurosensory disorders using neonatal US imaging bedside
2. Basic and translational research (leads: C. Mehats, G Kayem/T Schmitz, P Buffet)
- Identification of causal mechanisms leading to prematurity
- Maternal/paternal Exposome & predisposition to inflammatory disorders in pregnancy and preterm infants
- Prevention of perinatal infections
3. Innovation in data federation and clinical trials for effective care (leads: J Zeitlin, O Baud
- Creation of a data infrastructure integrating routine clinical data and research studies to improve perinatal care and health
- Innovative clinical trial on postnatal steroids to improve neonatal health
4. Parents faced with preterm birth: how to protect? (leads: C. Deneux, J. Wendland)
- Refinement of developmental care in very preterm infants
- Prevention of maternal somatic complications
5. Training, mentoring, and dissemination activities (leads: H Torchin, L Mandelbrot)
- Improvement of perinatal care through parental training
- Improve professional evidence-based training and practices

Over the next 5 years, FHU Prem’IMPACT aims to position itself as a major consortium for research and care in the field of prematurity. Numerous ambitious projects launched each year will be co-financed by donors committed to the cause of prematurity.
AN INTERNATIONAL DIMENSION
The consortium has opened up to teams in Europe and Africa, whether by joining international teams or participating in thematic research networks.
The Prem’IMPACT FHU aims to develop a specific research and development strategy for southern countries. IRD’s MERIT team operates in Benin and Ghana, and has a long-standing working relationship with FHU Prem’IMPACT member teams. In order to strengthen the impact of health research and interventions for patients in Africa, the Clinical Research Institute of Benin has joined the Prem’IMPACT FHU.
FHU Prem’IMPACT is also developing a training program with teams from CHU-MEL (Cotonou, Benin) and CHU de Saint-Louis (Senegal), enabling nurses and midwives to spend a few days to a month training with FHU Prem’IMPACT’s clinical teams. It is also developing ultrasound training courses accessible via the Internet in partnership with FUN (France Université Numérique). FHU Prem’IMPACT’s research teams have been involved in national (perinatal epidemiology), European and international research networks for many years.
